Week 1: (For girls 10-12 only) July 31st to August 3rd

Week 2: (For girls 11+) August 4th to August 10th

Please fill out and submit all required forms by July 14th. Anyone without the proper forms submitted in time will not be permitted to attend camp.

The Camper Health History Form comes from the state of Massachusetts, and we legally cannot keep any girl at camp without that form completely filled out along with a copy of a current physical form within the last 18 months. If you are worried about trying to schedule a doctor's appointment to get a physical, both Urgent Care and Minute Clinic offer walk-in summer camp physicals. Either is just as acceptable as a doctor's visit.

If this is your camper's first year attending RCMA, you will also need to fill out a photo release form, which can also be downloaded at the bottom of this page. Returning campers have their photo form stored at camp and do not need to fill one out again.

By July 14th, the following forms must be digitally through the link emailed to you:
-A fully filled out Camper Health History Form (linked below)
-A copy of the camper's most recent physical. This must be within the last 18 months from attendance to camp
-A copy of up-to-date immunizations
-A copy of your camper's insurance card
-A photo release form (linked below)
-Covid-19 Acknowledgment of Risk and Consent to Test form (linked below)
-Proof of Vaccination against Covid-19 if applicable
**Any camper that has not had these forms submitted by July 14th will not be able to attend camp this year.**

Please go over the suggested packing list well in advance to ensure your camper has all the necessary items for a week away from home.
